- Date(s) of event(s)
- Insurance coverage
- Number of attendees
- Purposes
- Organization name
- Profit, or non-profit status
- Responsible individual’s contact information.
- How many children and their age group if baby sitting is considered
We highly recommend you to have our minister for any non-denominational services, like weddings, memorial services, or birth celebrations.
